Important Information (Studio & Health Dept. Policies)
*Budo Studio does not tattoo or pierce minors.
*For those 18 years of age or older, government issued picture identification is required for each procedure (Passport, Drivers License, Regular State Issued photo I.D., Military I.D.)
*If you have an out-of-state I.D. , we need additional I.D. such as a College I.D., social security card, etc... (any type of I.D. you give us is confidential and will not be shared with anyone other than the health department.)
*If you have any heart condition that requires antibiotic treatment before any dental work, we require that you discuss this with your doctor before receiving a tattoo or piercing. Please stop by the shop or e-mail us for a medical form.
